The Communities and Patients We Serve

CMG clinics are the access point to primary care health services assuring that the CHRISTUS vision of creating healthy communities is fulfilled. The foundation of CMG’s mission is primary healthcare which provides continuing and comprehensive care for patients of all ages from infants to the elderly, for all medical conditions including management of diseases such as diabetes, high blood pressure and chronic health problems.

Primary Care

The scope of CMG’s primary care medical operations and programs include:

• Family Medicine
• Internal Medicine
• Pediatrics
• OB/GYN
• Geriatrics
• Clinics for the Underserved
• Our platform is a primary care model that includes, but is not limited to, the following services:
• Diagnosis and treatment
• Wellness and prevention
• Laboratory and radiology
• Vaccination and immunization
• Maternity services (pre- and post-natal including delivery)

Scope of services vary by clinic.

Health Promotion, Prevention and Wellness

CMG is experienced in providing a variety of health promotion and wellness services throughout our facilities. For example, some of the services provided at our facilities include:

• Preventive Care: Educational Services
• Wellness programs: Education, immunization, vaccinations and fitness programs
• Health risk appraisals
• Disease Management
• Health Education Programs

In various CMG health centers, health promotion and wellness initiatives range from Diabetic Disease Management, Well Child Group Visits with instruction in childhood development, nutrition and cooking classes, prenatal classes, ESL and GED classes, and exercise classes for patients.

CMG won the 2004 CHRISTUS Health Touchstone Award in the Clinical Quality category for our “Diabetes Management in a Hispanic Immigrant Population” program which was developed at our Southwest Community Health Center located in Houston, Texas.

CMG focuses on the total health and well being of the individual as a part of our healthcare ministry. This focus underscores the importance of health promotion and wellness on an equal footing with the treatment of illness and injuries.
